Job Description
The Case Manager provides professional-level service coordination and case management for individuals in MCHS programs. Requiring support and other resources. They are to provide emotional support to clients, while helping them structure their finances and plan their life goals along a structured program. Compassion and empathy are a requirement of this position.

- Build high-quality relationships with participants to understand the personal experiences of clients impacted by the community.
- Carry a case load of 14-16 clients and/or 15-20 hours of face-to-face time every week which includes males, females, and parents.
- Develop a plan with each client and coordinate with appropriate programs depending on need.
- Provide support to clients with locating appropriate resources, case management and direct client services to all clients who request services.
- Bachelor’s degree in social work or related field.
- A minimum of three years’ working in social work.
- Three to five years’ experience working with vulnerable populations.
- One years’ experience working within community systems, referring, case management.
